具体评审请求、事实、理由和法律依据应按《商标评审规则》和所附《注册商标无效宣告申请书》（正文样式）要求另附材料，连同此申请书（首页）一并提交。对部分商品或服务申请注册商标无效宣告的，须在所附材料中具体列明。此申请书（首页）及有关材料需按照对方当事人的数量提交相应份数的副本。
近期有很多客户在申请注册商标后，收到自称来自商标局相关单位的通知，通知其正在申请的商标因XXX原因被驳回了。而且该“工作人员”还能准确说出其注册商标的名称、类别、代理机构以及申请人的公司名称、地址、注册资金、经营范围等详细信息，有的申请人甚至随后收到《商标驳回通知书》。
